How do I submit a claim?

Submitting a claim for your pet is easily done through our online claim form.

Submitting a claim with PHI Direct is as easy as 1-2-3!


Step 1: Have your vet submit your pet’s complete medical record

If you have not done so already, please have your veterinarian send a copy of your pet’s complete medical records to claims@phidirect.com. Please note that we will need records from all the clinics your pet has been to.


Step 2: Complete the PHI Direct Claim Form 

Fill in all the fields on the claim form and provide all the requested additional information, which includes all invoices or receipts you were given at the vet. 

The easiest and fastest way to submit a claim with PHI Direct is by simply filling out our Online Claim Form which will be immediately sent to our claims team. You can also complete the PDF Claim Form and send it to us by mail or fax.


There are four ways that you can submit your claim to us:

  1. Complete the online claim form
  2. Download and email your claim to us at claims@phidirect.com
  3. Download and fax your claim to us at 1-866-428-6063
  4. Mail your claim and receipts to us at

Step 3: We’ll review and process your claim

Let our Claims Team take it from there! If our team needs any further information, they'll reach out to you directly.
