Before you can submit a claim with PHI Direct, we will need your pet’s full medical history.
To submit a claim and get it processed, we need your pet’s full medical history, which includes any vet clinics your pet has been to in their lifetime.
While it is not required before you submit your claim, we will need your bank information, if you haven’t provided that already, so we can set up direct deposit to pay you for eligible claims. Not having your direct deposit information could cause a delay in your claims reimbursement. Learn more on how to set up your direct deposit.
To get your pet’s full medical history, please contact any vet clinics your pet has visited and have them send us your pet’s records to claims@phidirect.com.
